Forms are created in PDF format and require Adobe Reader, versions 4.0 and above to view them. If you wish to e-mail a form to another party, simply click on the Adobe icon that appears on the far right side of the document of interest. This will start the Adobe Reader application which has an e-mail button in the toolbar.
